All she had to do was say it was a mistake and she regrets that the record doesn’t state what she recalls, saying something about how she is glad she also has it recorded correctly in another area in the note (the unchecked pressured speech box in the mental status exam). The problem is that she is combative and making it HIS problem rather than taking responsibility.
