The thing is as a medical professional what you put in a chart follows a patient and other medical professionals might see their history and make choices that hinder the patient based on that medical history. so if you put false information or even make mistakes without correcting them you could be causing irreparable harm whether you mean to or not.

I predict that she will be looking for a new profession, because there is no way in hell that anybody in their right mind would go and see her. Her response would have pissed me off, because she was defensive over her mistake. Saying that I don’t care what it says that she knows what she meant, she might have known what she meant, but nobody else would have and I’m sure if she went back and read that message in a couple of months. She probably would just be like oh yeah, the patient must’ve had pressured speech.

You’re a medical professional you don’t get to say I know what I meant when it’s regarding someone’s health care. In any type of court record what if “ oh the court wrote this but that’s not what we meant” she is the farthest thing from an expert in anything.

It’s bad enough that she is a doctor and doesn’t understand the importance of proper punctuation so that her clinical notes can be interpreted correctly. She could have just used, “Not hyper; not pressured speech,” or, “Patient did not exhibit pressured speech.” The fact that she says, “I don’t care what it says. I know what I meant,” is WILD.
