@alanonai: Here's a nice thing about building software: some of the decisions you'll feel worst about were actually good decisions. Say you're building an app and you have to choose what happens when two people edit the same thing at once. You pick the simple option, because you have forty users and two of them being in the same place at the same moment basically never happens. That's the right call. You'd make it again. Then a year later you're getting support emails about work disappearing, and you feel like an idiot. But nothing broke. Nobody made a mistake. The code is doing exactly what it always did. What changed is that the assumption underneath it, that simultaneous edits are rare, quietly stopped being true one ordinary Tuesday. Your decision didn't fail. It expired. That's a different problem than a bug, and it needs a different skill. Not being more careful up front, because there was nothing to get right. Just noticing when the ground moved.
