They are misinterpreting her note on purpose to try to prove that she had symptoms of mania and it was missed. Pressured speech can be a symptom of mania. I don’t agree with how she responded, but her response isn’t what’s evidence for the trial… it’s her notes and what she wrote in the medical record. Her documentation is actually correct. In medical school we are taught to document a negative review of systems the way that she did. The word “No/Not/Denies” and a list of symptoms followed by a coma. For example: No chest pain, shortness of breath. A physician reading this sentence knows that the No applies to each of each symptoms listed. Also, if it were a positive symptom it would be in a separate sentence followed by all the other positive symptoms. They are in fact misinterpreting her note. The medical record is meant to be interpreted by people who are taught how to read and write in a medical record.

So many doctors in the comments saying well this is how we were taught how to document. That is why support staff have to go around explaining and interpreting your fucking notes all damn day long. Get it together!!!

Yeah people have tried to shame me for how I document at mental health facilities and I feel very validated right now. They can all kiss my 🍑 and I will continue documenting exactly how I have been, thoroughly!

There is a significant difference between a nursing note and a medical note from a psychiatrist. So, to make the comparison is ridiculous.
