This is inserted into the same note, along with the unstructured data(free text). These templates are created for the users ease but at the end of the day, the provider is responsible for reviewing the note before finalizing it. For continuity of care purposes this is part of patient care. It's absolutely imperative to document accurately and completely. I'm curious from a med school standpoint what education the providers receive regarding documentation and if the legalities are truly stressed enough. Thank you for sharing your perspective from a providers view! This is a huge issue with documentation.
