here is the secret. notice the hole isnt a round hole. there is a pin for the cap to hit. when it hits the mechanism circled with yellow color pushes out the other part of the pen.

Because the blue cap blocks the top of the bottom sude view when the spring bends the opening of the device blocks the space when the hole opens the side of the cap the pen removes the back side... Now You Know. 🤣👍🤗😆😵‍💫🤔

the cap slides down the pen. when the clip on the cap enters the box, it pushes the little end piece out the other side. you're welcome.

The insert end is connected to the outer edges which connected to the exit end by the outer edges. Inside there is linkage that is simple but invisible. 2 narrow rods at the door hinge and the other side.
