I know a nurse who works in the ER, she would stay past her shift to finish her charting. I asked her why she didn’t do it when she came back to work later that night? Her response, “I can die on my way home or be in an accident and get a brain injury and my patient would be left with half ass charting.” Yes, it is that serious for her. Top notch nurse

As a Psych nurse I have always put things in parentheses. A patient stating “Fuck you, you stupid fat ass Cracker. I will Fuck you up”… is totally different than: patient cursing at staff. 🤷♀️🤦♀️

As a physician (ED- Brazil), my fellow colleagues tend to say I’m too into details because I write EVERYTHING in the medical records... But seeing the nurse and the doctors of this trial, I feel sooooo glad to be able to respond in that kind of details as THAT nurse! She’s a real badass!! That was a true lesson!!!

They prescribed me the wrong medication ( actually one she was on). It made me not sleep, talking compulsively, irrational and angry. I took it for about 2 weeks and legit felt like I was going insane. Told them hey we need to switch because I’m screaming and crying at people. They looked back and in my notes in all caps it said “ DO NOT PRESCRIBE AGAIN “ they went oh whoops. I’m sitting there shaking and with about 3 hours of sleep in the last week. Just want to throw that out there

Me making a note that said “MD entered the room, approached the patient and proceeded to palpate the patients abdomen. Patient grimaced and retracted upon palpation. MD stated “OH that’s fine” and exited the room.” Patient was taken to OR for bleeding that evening.
