@ivleaguetutoring: Can you figure out all the things that went wrong here on this nursing NCLEX Case Study? **These NCLEX case studies are created and not taken from real patient scenarios. They are teaching and learning tools for nursing students and are dramatized because I am extremely dramatic and did theater in high school** #nclex #nclexcasestudy #bloodtransfusion #nursingschool #nextgennclex
You can run for an hour but at least start on slow rate for 15 minutes

There is nothing right 😳 I'm shocked first O+ and O- yes they are both O but the donner has a rhesus will the reciever no so you're introducing a foreign body to the patient which gonna reject the blood. If the patient was O+ and we gave O- it would have worked because you're giving O but not introducing something new. Second of all when we transfuse blood we stop all of the other IVs except for normal saline cause this D5W will letting the rbc burst. Third of all a transfusion time is expected to be ranging from 1:30 to 4 hours so 1 hour is too fast which can cause tachycardia and vasodilation I think and why the fuck didn't she used Y tubing? the transfusion rate of 250 mL/h with a Volume To Be Infused of 250 mL is completely normal. But, What the fuck does her hgb so low! 1 unit actually is expected to raise the hbgb by approximately 1.0g/dL

re pump: rate’s too fast. there’s more than 250 mL in the bag. 1 hr transfusion time is also too fast.

thought, it indicated 0 drop transfused, per the patient's blood type with rhesus factor being negative patient needed 0- Not 0+ and the mL/hrs for the drop rate should've been 250mL/4hrs and 250mL/1hr.
