What do you do when doctors write something in their notes that did not happen? For example I had an appointment with GYN and when I went into my chart to see my visit notes, the doctor put that she had discussed the benefits of an IUD to help with my current symptoms, she never mentioned anything about an IUD to me 😩 she also put she discussed with me about nutrition and exercise (my BMI is high) and she never did that either 🥺

we were talking about this in our work meeting (medical biller). One of the issues with EMI is the use of templates and cut and paste chart notes leads to inaccurate charting, and using AI to chart is making it even worse.

Yes! as a former nurse this was drilled in us in school. Document everything in detail or it did not happen and u will not remember years later if u are called to testify in court.

I was an X-ray tech. I was taught that my notes needed to be clearly understood by anyone who would read them cause I was almost never going to be in the room with the radiologist to explain what I wrote.

I work in big ag accounting. If I didn’t record it, it didn’t happen and we’ll have to give you a piece of equipment worth millions. I’ll be fired on site. It should be more imperative when it’s a HUMAN BODY.

In addition to my actual day job, I'm also a notary.. which really carries pretty low stakes, but I still document the hell out of every notarization. Why? because I could be subpoenaed years later... and trust me, I don't know what I had for breakfast this morning. I'm certainly not going to remember the details of a document that I attested to Susan signing 3 years down the road. I can't even imagine health professionals with so much more on the line could be that careless.
