My cousin in Nebraska had something like this. His wasn't for use, it was so that if there was a sewage backup it would catch it before it got into the house. The underground toilet was cheaper and easier than installing a professional sewage overflow trap.

I looked into this recently, I guess its because they would rather sewage spills from backups caused by poor infrastructure to overflow out in that space rather than anywhere else in the home. Toilets were used because they keep sewer gasses from escaping.

I know the reason and it's stupid. so don't read this if you hate stupid reasons. More than likely this Pittsburgh toilet is to act as a low-point overflow drain that kept sewage backups out of main living areas. it's unlikely that this toilet was being used for any sort of bowel movement given the lack of room above it. Thank you for coming to my Ted Talk.

I had to look twice at it and I believe this is a T junction. The pipe from the toilet runs straight and downhill and another pipe joins it. If it did run uphill the crawl space would be filled with methane gas!
