I felt that way for 3 months but it went away. I was actually drinking almost a gallon of water per day during that time and I felt the same (extremely wired) I felt maybe my sugar levels were coming down quickly; A1C from 6- 4.7.

Here’s my theory on why GLP-1 results and side effects vary so much, and why we're hearing more about extreme fatigue and anhedonia (emotional flatness): It comes down to optimal levels of both estrogen and testosterone, not just as 'sex' hormones, but as essential, whole-body regulators of our entire metabolic, nervous, and dopamine systems. GLP-1s lower hunger partly by dampening dopamine in the brain's reward centers. That turns off 'food noise,' but if system-wide hormones aren't optimal, it can turn down the volume on everything, leading to anhedonia, loss of drive, and severe fatigue. Premenopausal women with optimal estrogen and balanced testosterone have a full hormonal shield, leading to higher weight loss, sustained energy, and joint protection. Perimenopausal/postmenopausal women (often around 47+) experience a drop in both estrogen and testosterone. Adding a GLP-1 caloric deficit on top of low system-wide hormones deprives the brain and joints of key regulators, triggering severe exhaustion, mood blunting, and joint pain. (Which is why adding HRT to a GLP-1 yields significantly better results!) Men need optimal testosterone not just for muscle, but because it converts to estrogen and directly fuels dopamine drive. When their testosterone is low, GLP-1s are much more likely to cause mental flatness and fatigue. Bottom line: GLP-1s don't operate in isolation. Baseline whole-body hormones like estrogen and testosterone act as the engine that protects your energy, mood, and overall response on these medications.
