This is the blood test I wish more women had checked before IVF. And it’s not AMH. Most fertility investigations understandably focus on reproductive hormones like AMH, FSH, LH, oestrogen and progesterone. But there’s another marker I pay close attention to when someone is preparing for IVF: fasting insulin. Insulin is an important metabolic hormone, and when levels are chronically elevated, it can influence ovarian function, androgen production and ovulation. Insulin resistance can also be associated with increased inflammation and oxidative stress, which are important considerations when we’re looking at overall reproductive health. And this isn’t only relevant if you have PCOS, diabetes or are overweight. One of the biggest misconceptions I hear is, “My fasting glucose is normal, so my insulin must be fine.” Not necessarily. In the earlier stages of insulin resistance, your pancreas can compensate by producing more insulin to keep blood glucose within a normal range. This means glucose can look perfectly fine while insulin is gradually increasing in the background. That’s why I don’t like making assumptions based on someone’s weight, menstrual cycle or glucose result alone. Fasting insulin certainly isn’t the only factor that determines IVF success. Age, ovarian reserve, egg quality, sperm health, genetics, embryo development and overall health all matter. But when someone is preparing for IVF, I want to know whether there are metabolic factors we have an opportunity to address before treatment begins. Those months leading into IVF can be incredibly valuable. They give us time to investigate properly, address nutritional deficiencies, support metabolic health and work on the factors that are actually modifiable.
