Why do I still have pain after endometriosis surgery? This is something we hear all the time, and persistent pain after surgery does not automatically mean your surgery failed. Endometriosis surgery can remove lesions, adhesions and scar tissue, but surgery doesn’t necessarily remove every possible driver of pelvic pain. Pain may still be related to endometriosis that remains or recurs, adenomyosis, adhesions or scar tissue, pelvic floor dysfunction or muscle spasm, bladder or bowel involvement, ongoing inflammatory processes, ovulation or menstrual pain, or changes in the way the nervous system processes pain. That last one is particularly important. After years of persistent pain signalling, the nervous system can become sensitised, meaning pain pathways become more responsive and can continue amplifying pain even when the original tissue driver has been treated. And often, there isn’t just one mechanism occurring in isolation. This is why treating endometriosis and treating persistent pelvic pain are not always exactly the same thing.
