In a 2026 study, 3 of 4 long COVID patients had a normal nerve biopsy. Then researchers counted the autonomic fibers, and those were damaged even in the normal group. These tiny fibers do two jobs. They carry pain, and they run your heart, gut, and sweat. So when they start dying you can get pain, a racing heart, and a stalled gut. The same damage shows up in about half of fibromyalgia and a big share of POTS. Not everyone has it, and the standard biopsy misses the autonomic fibers, so a normal result isn’t the whole story.
