@doctorhaleymain: Not every patient with low back pain needs prone press-ups. When your patient has their greatest pain with sagittal-plane movements and also demonstrates positive dural tension signs, that pattern may increase your suspicion for discogenic or nerve-root involvement. That’s when I start thinking about axial separation. The goal is to use distraction to unload the involved segment, decrease compressive forces, and assess whether symptoms improve. Then, if the patient demonstrates a clear extension preference or centralization with repeated extension, prone press-ups may be appropriate to add. The key is not choosing a technique because it’s popular. It’s choosing the technique because the exam gave you a reason to. 🧭 Clinical reasoning first. Technique second. On the side lying, try having top leg going towards the side of the edge of the table, and you could change your distal contact to the knee, there’s no need to go skin to skin on the greater trochanter, it’s a stretch
