When a job offers health insurance as a "benefit," most people picture their boss generously paying for it. That is not what is happening. It is your money. They are just spending it for you. Here is the real structure. The average employer family health plan now costs about $27,000 a year. Your employer writes most of that check, but that money is still part of what it costs to employ you. It is compensation. It just never touches your bank account, it gets routed straight to an insurance company on your behalf. Economists have measured this for decades. One JAMA study found that health-premium growth from 1988 to 2019 cost the average worker roughly $8,000 a year in wages they otherwise would have taken home. You may not have gotten a raise this year. Your insurance company did. So the real question is not whether insurance is a nice benefit. It is whether the company spending $27,000 of your money every year is any good at spending it. And they are not. Watch how badly they shop. A prescription comes in listed at $22,000. The insurer "negotiates" it down to $7,000 and pats itself on the back for saving $15,000. Fill it three times and they will tell you they saved you $45,000. Incredible work. That same prescription is $220 cash at my pharmacy. Two hundred and twenty dollars. They did not save you $45,000. They overpaid by almost $20,000 and called it a discount. And because it was your compensation footing the bill, you are the one who lost. They wasted your money, not theirs. That is the whole problem. The people spending your healthcare dollars feel nothing when they overpay, so they overpay constantly. Private plans routinely pay multiples of what the cash price or the Medicare rate would be, because there is no reason for them not to. That should make you curious. It made me curious.
