Its commonly misunderstood as Goku making a dumb decision for not taking the medicine but Goku was actually supposed to only take the medicine when he gets symptoms it’s a cure not a preventative he never showed symptoms of the heart virus so he never needed to take it, think of it like any other treatment you take when your sick not when you healthy

The reason Goku’s heart virus took longer to act up is because Trunks killed Frieza. Trunks thought Goku wasn’t gonna make it in time bc he doesn’t know Goku learned instant transmission. Goku sped up the heart virus by fighting, so in this timeline he triggered it by fighting the androids and it showed up later instead of sometime after the Frieza rematch. The time travel logic in this saga is REALLY good, and very important to understanding the characters so please if something is confusing think it through and find out the answer!! It’s what takes this arc from truly great to the best of Dragon Ball for me.

The DB manga is the best way to experience the story of Dragon Ball and I'd die behind that, I love the anime but there was way too much subtle but noticeable mischaracterization and story changes in the anime.
