For anyone asking a sentinel event is when someone passes or will suffer long term damage from something unnatural in a healthcare setting. Also, potassium can send people into cardiac arrest or worse

I watched a surgeon go to work drunk every day for 3 years without consequence, and they were covering for him when he removed 15 inches *too much* of someone's intestine. So if it was a physician nothing surprises me anymore.

It doesn't really matter if pharmacy sent down potassium. It's up to the person giving the drug to check what it is in the vial. the 5 rights of medication administration just forgotten 9 times!

I worked at a hospital. My dad was a patient there and they let him bleed out from a misplaced iv. He was a full code on the heart wing. I found him dead before my shift early in the morning and the hospital stance was no he didn’t bleed out. Even denied the photos I took.
