@derekweyhrauch_md: 99 problems but LDL ain’t one. - Lifestyle * Diet, exercise, weight loss, soluble fiber, reduced saturated fat. * LDL is strongly genetically determined, so lifestyle has limitations. * Typical LDL reduction: 5-15% * Still foundational for overall cardiovascular and metabolic risk reduction. Ezetimibe * Blocks intestinal cholesterol absorption via NPC1L1. * LDL reduction: 15-25% * Once-daily pill; inexpensive and generally well tolerated. * Commonly used as add-on therapy. Bempedoic acid * Inhibits ATP citrate lyase upstream of HMG-CoA reductase. * Activated primarily in the liver rather than skeletal muscle. * LDL reduction: 18-25% * Useful for statin intolerance. * CLEAR Outcomes demonstrated cardiovascular event reduction. * More expensive than generic statins. PCSK9-targeting therapies * Alirocumab, evolocumab, inclisiran * Increase LDL-receptor recycling or decrease PCSK9 production. * LDL reduction: ~50-60% * Injectable; monoclonal antibodies generally every 2-4 weeks, inclisiran twice yearly after loading. * Highly effective but more expensive. Combination therapy * Combining therapies with different mechanisms can lower LDL by >70%. Statins * Atorvastatin, rosuvastatin * Inhibit HMG-CoA reductase and increase LDL-receptor activity. * LDL reduction: 30-55% * Can lower hsCRP ~20-40%. * Strongest evidence for reducing heart attacks and strokes. * Generic and inexpensive. - #doctor #medicine #health #wellness #prevention - Disclaimer: The contents of this video are for informational purposes only and are not intended to be medical advice, diagnosis, or treatment, nor to replace medical care. I reduced my LDL 36% in 4 months. Increased my cardio to 150 minutes per week, cut out almost all processed foods and got 10,000 steps a day. Lost 16 lbs as well.

Please discuss the increase in cholesterol as women enter menopause. God makes no mistakes. Our cholesterol increases as estrogen decreases. My cholesterol is 305 (genetic) but I will not go on a statin. A statin almost killed my father. Severe muscular pain and. pancreatitis. Now needs a walker to walk.

lessons from mom :
Statins are garbage. I tested positive for lp(a), a genetic high cholesterol that doesn’t respond to diet/exercise. I use a pcsk9 inhibitor with a glp1 to reduce inflammation even though I didn’t qualify by weight. My cholesterol was alarming and they felt I needed to take quick action.

I take ezetimibe by itself and my cholesterol went down about 50 points. No side effects. Bempoic acid is hundreds of dollars per month.

Lp(a) reduction … isn’t this what really matters (vs ldl)? I’m confused. And how to reduce lp(a) since it’s so genetically determined?

Great info! When should we schedule with a preventative cardiologist? Or can a PCP manage high cholesterol with statins?

40mg Atorvastatin with muscle aches that are getting hard to handle. Crestor any better for muscle aches? If I ask my Md. to half to 20 mg what would be my best second option before paying for PSK9?

Any thoughts, I took 5mg Crestor 3xs a week. In about a month my energy crashed hard and my insulin went up significantly . Stopped and everything resolved. It did great for my cholesterol #s but can’t live like that.
