If your ferritin or iron markers aren’t improving, one of these five issues may help explain why: 🩸 **You’re losing iron faster than you can replenish it** An iron supplement can’t always keep up with ongoing blood loss from heavy periods, endometriosis, gastrointestinal bleeding or another untreated cause. You may absorb some iron every day while simultaneously losing even more, leaving your levels low despite being consistent. 🔍 **You’re not checking the elemental iron** The large number on the bottle may represent the weight of the entire iron compound—not the amount of actual iron it delivers. For example, 325 mg of ferrous sulfate typically provides approximately 65 mg of elemental iron. On the Supplement Facts panel, look for the number directly beside “Iron.” That is the elemental amount that matters. 📉 **It’s a maintenance dose—not a repletion dose** Some multivitamins, prenatals and general wellness supplements contain iron to help support normal daily nutritional needs. That doesn’t necessarily mean they provide the amount your clinician would recommend to rebuild severely depleted iron stores. The appropriate dose depends on your labs, the cause of your deficiency, your tolerance and your medical history. ⏳ **It’s slow-release and may miss the best absorption area** Most iron absorption happens near the beginning of the small intestine. Some slow-release, delayed-release or enteric-coated supplements are designed to dissolve farther down the digestive tract. This may make them gentler on the stomach, but it can also mean less iron is available where absorption is most efficient. 🧲 **It’s combined with competing minerals** Calcium can interfere with iron absorption, which is why iron and calcium supplements are commonly taken at different times. Large doses of zinc or magnesium taken in the same serving may also create mineral competition, although the evidence is strongest and clearest for calcium. Check whether your iron is part of an all-in-one mineral formula, and ask your doctor or pharmacist whether separating the doses makes sense for you. If your levels aren’t moving, don’t automatically double your supplement. Bring the actual bottle, your dosing routine and your latest CBC, ferritin and full iron panel to your healthcare provider.
