Shoring Foreman, hell, the whole shoring crew getting fired! This is what happens when the foreman, the general foreman, the superintendent and the quality control person doesn't walk the job before each pour along with the General contractor walking it and having a quality control sign off sheet. To me it looks like they didn't brace the shore post every so often with a square coral bracing and the post slid with the weight and vibrator. Carpenter better throw a CJ all around that perimeter of the bust so the rest of the deck can pour and just fix that shoring. I didn't see any tension cables so they're lucky about that
