This has actually happened to me! I did not go into V-Tach though. It was a wild ride for a few minutes cause the nurse panicked and pressed the code blue button when I couldn’t breathe. Seeing the code team from the other side is wild.

I had a patient that coded like 1 min after I gave IV metoprolol, I still get nauseous when I have to give it, it’s been 4 years. It wasn’t a med error, I can’t imagine if it had been someone I know or if it had been an error.

Same happened to me. I was having the anaphylactic reaction. 1 RN placing IV other getting meds. The one getting meds pulled up code dose epi 1mg into syringe. When she gave them to nurse she started giving obviously. As she pushed the first med I immediately knew something wasn’t right and asked what she was giving because I didn’t feel right. She said Benadryl. It makes you feel funny sometimes. I remember distinctly saying no no that’s not Benadryl I think I’m dying. Monitor went off svt 210s. And the nurse said my name like 2-3 times apparently I didn’t answer but remember hearing her. I then heard oh f*** and that was it. I woke up sometime later and has 10 people in room with classic MI symptoms. Turns out that it wasn’t Benadryl it was epi. I went from svt to vt, and shocked 1 time and converted back. And was having a NSTEMI. 😳

This happened to me at age 16, the doctors put in the orders incorrectly & was not clarified by nursing before administering . IM Benadryl (instead of IV) & IV Epi (instead of IM)

Honestly… I almost did that to a patient like 15 years ago… worst feeling ever! Working nights and it was like 4-5am in the morning… pregnant etc. tired. Thankfully my co-worker caught it before I pushed it! I’ll always remember that patient! Of course I wrote myself up.

As a baby nurse (40 years ago) I was pulled to a different floor. Grabbed potassium, where I thought was normal saline was and tried to dilute the antibiotic. That god the antibiotic didn’t dissolve. I easily would have killed the patient.

Had a coworker tell me one of his friend’s family members horrible experience. Told the triage RN about med allergies (Ibuprofen was one of them) but it wasn’t put in the pt’s chart. Dr ordered something for pain once the pt got to the back. Nurse gave meds to the pt without telling them what it was. It was ibuprofen and the pt went into full on anaphylaxis and had to be intubated, etc.

I had something similar happen. No heart attack thankfully “that I know of” I just know when I came to I saw the crash cart and an ass ton of people around me. I later received an apology from the nurse that she pushed Epi instead of IM………..
