The pronator drift. Ten seconds, no equipment, and it’s one of the first things I do when I’m worried about a stroke or a mass. Pronator drift is a highly specific but relatively insensitive test for detecting subtle corticospinal (upper motor neuron) weakness, so it is useful for confirming a lesion when positive but poor at ruling one out when negative. A clear positive is meaningful; a negative does not exclude subtle weakness. We combine it with more sensitive maneuvers — forearm/finger/thumb rolling, rapid alternating movements, finger tap, and reflex asymmetry.  A drift with pronation reflects corticospinal (pyramidal) dysfunction, whereas an upward/lateral drift without pronation suggests cerebellar or proprioceptive dysfunction — the pattern helps localize but is not diagnosis-specific.  To be clear, this is one exam finding, not a diagnosis, and nothing here is medical advice. Arms drift for boring reasons all the time — a cranky shoulder, fatigue, tight neck, or just doing it wrong. A single drift on your kitchen floor means very little on its own. What matters is context: if it’s new, or it showed up alongside slurred speech, a facial droop, numbness on one side, or sudden vision change, that is a call to 911, not a comment. If it’s been like that for years and nothing else has changed, bring it up at your next visit and let someone examine you properly.
