The Pitt Season 2 Episode 5: Langdon insists that when he treated Debbie earlier that morning, she had shown only uncomplicated cellulitis and that prescribing oral antibiotics had been reasonable. Robby, still frustrated with him, takes control of the case while Langdon stays with Debbie and tries to reassure her. Debbie deteriorates quickly. Her temperature rises to 102, the redness continues racing up her leg, and a fluid-filled bulla develops. Robby becomes increasingly concerned about necrotizing fasciitis, an aggressive infection that destroys tissue. Debbie is frightened when surgery is mentioned, explaining that she had thought the problem was only minor swelling from being on her feet at work. Soon her blood pressure crashes to 85/40 and her oxygen saturation falls to 89%, indicating septic shock and respiratory failure. Robby supports her breathing while urgently waiting for a surgeon. Instead of an experienced surgeon, Rick Mosely arrives and admits that he was a medical student only weeks earlier and has never personally treated necrotizing fasciitis. Robby is furious. He explains that Debbie’s infection was confined to the top of her foot only hours earlier but has now spread almost to her knee, with swelling, bullae and other signs of rapidly dying tissue. He sends Mosely away and demands someone more experienced. Dr. Yolanda Garcia finally arrives but wants a CT with contrast before operating. Robby argues that Debbie cannot afford the delay because the infection is advancing too quickly. When Garcia still wants confirmation, Robby performs what amounts to a bedside surgical test himself: he makes an incision into Debbie’s infected leg. Gray, dirty-looking fluid immediately emerges and the fascia appears gray, confirming his fear of necrotizing fasciitis. Garcia realizes Debbie needs surgery immediately. Debbie is rushed upstairs to the operating room for emergency surgery, ending her storyline for the episode with her life, and potentially her leg, still in jeopardy.
