I think we have a huge gap in therapist training when it comes to suicidality. Many therapists are trained with a liability lens, avoid getting sued, do no harm = make sure no one dies on your watch, which it’s not it.

I have to tell you, this tone is not my favorite. Sure, maybe he could’ve been assessed further and maybe something was missed. On the other hand, clinicians also have to be sure the client is safe. Clinicians do not have three hours to triage either. Your ER may have been thought to be the safest place in that moment.

Psychologist here. People with OCD also complete suicide…. Just because the diagnosis may behave not been correct, that’s irrelevant for whether the kid needs to go to the ER. The counselor did their part, there’s no need to be hateful towards other providers. We’re all in this together.

well...as a licensed mental health therapist, yes we can assess for OCD but I will still break confidentiality if a patient tells me they have suicidal thoughts or they want to hurt themselves, we are damn if we report it and we are damn if we don't, so I rather be safe than sorry, so I'm sorry that I have to waste your time at the ER to take care of this, that is part of your job which you are getting paid for, and I certainty don't appreciate the way you are delivering this message to all of us clinicians, which we are overworked as well. What makes you think these questions weren’t asked? We do use our brains, our education, and our experience. These questions are asked. We don’t immediately hear those trigger words and stop there and tell them to go to the ER. We use things like the Columbia Screener and ask very in depth questions. Have some faith and respect in us to do our jobs, just like you ask of us to do ours. What really pisses me off is completing a risk assessment on a student where they tell me they have thoughts and a plan and intentions, I send them to the ER and then see them in school the next day because a clinician didn’t find them to be of harm to themselves. Would love to know how that happens.

As a therapist, the weaponization of fear of losing our license/career has become so central in the discussion of suicide that we’ve lost our actual clinical judgment. Colleagues hear suicide and stop using any sense. It’s sad - for our field, but ultimately disparages the relationship and outcomes for our clients. I refuse to work this way

As a therapist, there is a huge gap in training and knowledge about OCD, which is why it can take 15yrs to get diagnosed with it. While I agree this training needs to more accessible to provide better patient care, and there needs to be more training in understanding and dealing with patient SI, I don’t think it’s necessary to generally state that we need to “use our brains.” If you want to speak generalized statements, maybe addressing the education gap would be more effective.

To be fair I don’t know how many therapists are adequately trained in OCD unless they specifically work with OCD. I know I never learned anything but a cliff notes version outside of CEUs. That being said, I also wouldn’t refer to ER etc unless they sounded like they had an actual plan to carry out.

THANK YOU! I don’t understand why practitioners won’t just ask a few more questions and conduct the safety assessment themselves. especially if you are a licensed social worker, psychiatrist, or psychologist in outpatient setting. That is your job..

As a clinician who worked with kids and teens for 10 years, I’d do a full assessment for intent and plan before sending kids to the hospital. I cannot count how many times they got sent home because they changed their story and self-reporting to the folks assessing at the hospital.

Unfortunately crisis intervention is not a skill set that’s well developed for therapist who went from school to direct practice. I started my career as a crisis counselor and was shocked at how inept my therapist colleagues were at assessing for suicide.
