I provide activities in a carehome, I've started in new one and all notes at the moment are task based, carestaff think activitie teams should document any meaningful engagement, there really needs to be a culture change and I've found this in lot of the care homes I have worked in.

in my experience, if staff spend as much time writing the care notes for every single interaction they have with residents there would be no time for the actual interactions with residents.

Fantastic highlight . Do you think having inconsistent carers on a run contributes to this quality of care note ? We have observed in our line of work , consistent carers are familiar with client care plans however on instances where runs are altered , no practical handover is done between the new carer going into the call and the client so then this is expected in their notes .
