Great overall info and appreciate your posts, but I question the usefulness of PPD test in this scenario. As screening test, PPD (and IGRA) has little diagnostic value in pt w/ highly suspected TB. And you have to wait 2 days for reading, so it's not really "quick" either. Even then, positive reading still requires f/u imaging. In this case, as you mention, prioritizing obtaining sputum for AFB smear and culture and getting CXR (or better yet CT) seems more appropriate management. Disclaimer: I'm not a nurse and not sure how nursing protocols differ. Just wondering why even bother with PPD at all.

My mom has advanced lung cancer and she had taken care of her mom who had advanced lung cancer. The coughing up blood, the being a smoker, paired with the swollen lymph nodes made me think advanced lung cancer!
