@aircrashbriefs: In October 1996, TAM Transportes Aéreos Regionais Flight 402 (a Fokker 100) was taking off from Congonhas Airport in São Paulo, Brazil. Just seconds after lifting off the runway, an electrical fault caused the right engine's thrust reverser to suddenly deploy in mid-air. The aircraft's built-in safety systems correctly reacted by automatically pulling the right throttle lever back to idle to prevent a fatal asymmetrical thrust. However, the pilots were completely unaware that the thrust reverser had opened, because the aircraft's systems failed to clearly warn them of the specific failure in the cockpit. Believing they were simply experiencing a minor auto-throttle glitch, the co-pilot repeatedly tried to force the right throttle lever back up to full power. In a tragic struggle against the plane's own safety mechanism, the co-pilot pushed so hard that he physically snapped the internal safety cable. This sent maximum takeoff power directly into the reversed right engine, while the left engine was still pushing forward. The extreme aerodynamic imbalance caused the jetliner to roll violently to the right, plunging into a densely populated residential neighborhood just 25 seconds after liftoff. The crash killed all 95 people on board and four people on the ground. This devastating accident revealed critical gaps in pilot training and aircraft design.
