Is methylfolate actually better than folic acid? There’s a lot of misinformation around this conversation, particularly when it comes to MTHFR. Folic acid and methylfolate are both forms of vitamin B9, but they’re not metabolised in exactly the same way. Folic acid needs to move through several steps before eventually contributing to the biologically active folate pool, including 5-MTHF. Methylfolate is already in the active 5-MTHF form and bypasses the MTHFR conversion step. And this is where MTHFR gets misunderstood. Having a common MTHFR variant does not mean you cannot process folic acid. Certain variants can reduce enzyme activity, but that’s very different from having no ability to metabolise folate. Folic acid also has the strongest body of evidence for preventing neural tube defects and remains recommended in Australian preconception and pregnancy guidelines. So this isn’t about saying methylfolate = good and folic acid = bad. In clinical practice, I often prefer an activated folate form when choosing a prenatal or standalone folate supplement, depending on the individual woman and her needs.
