The kits that are prepared in the main Pharmacy by a pharmacy technician. The kits are reviewed and signed off by a pharmacist.

At my hospital, the RNs pull the vial from the Pyxis for the anesthesiologist. If it was in its vial there shouldn’t have been a mistake as the RN and MD would check the vial. Very sad.
