Please stop trying to describe it like there is a way to make it make sense. Going to a hospital is like going to a restaurant, eating a meal, then walking out to find that the restaurant and their insurance company have laid claim to your house, sold it, razed it, and the is now an interstate running over your land.

patients ask me how much something is going to cost (inpatient) and I usually say "more than it should tbh" I'm openly critical of insurance companies to patients and it legit build rapport lol

Plus, it’s health insurance. So they may print a menu say thing they’ll pay $90 of your $100 cheeseburger. Then when the bill comes, they’ll just decline to pay anything. Insurance companies are the nasty skidmarks in the tightly-whiteys of society.
