Orihime currently rejected yhwach’s existence which removed herself aswell, that was the intended plan of her sacrifice, but uryu indirectly saved yhwach and orihime by killing jugram off, the power fully went back to yhwach and he now will pick another timeline where orihime will not have the opportunities to fight him, it was all the result of ichigo’s new bankai so in this new timeline yhwach will pick, he will make sure to break ichigo’s bankai first like in the manga. What happened right now did happen, but yhwach has the power to change the future and will do so. So basically this didn’t really happen. Orihime is still alive and will still be having their child kazui with ichigo in the end.

To punish Ichigo, she decided to choose future where Ichibe Hyosube chop Ichigo into tiny pieces - including cutting off his di*k. That's why she rejected Yhwach. If there is no Soul King, Royal Guard would cut off Ichigo's arms, legs, heart, balls... and turn him into imprisoned doll for all eternity. That's how Orihime always wanted it after Isshin Kurosaki failed to heal her dying brother. Revenge on Kurosakis family was always her ultimate goal.
