Pharmacy sent up an insulin pen through the tube system with a label of Lantus and a dose of 80 units. I scanned the label and was inches away from administering when I noticed a sliver of the original label under pharmacy’s label. It was a dark purple color and Lantus is usually lavender. You know what is dark purple? Novolog. I ripped the pharmacy label off and almost fainted. I peel back those labels every single time now.

I understand this situation is terrifying, but turning it into “how can we trust pharmacy now?” is a little much. One catastrophic pharmacy error does not mean every medication pharmacy prepares should suddenly be treated as suspicious. Healthcare depends on us trusting each other to do our jobs. Nurses administer medications they didn’t prepare, pharmacists verify orders they didn’t prescribe, and pharmacy techs compound medications that pharmacists then verify. There are checks in place for a reason. What happened in Nashville was horrible and absolutely needs to be investigated, but it was also an extremely rare event. Making people afraid of every pharmacy prepared medication isn’t the answer. We’re all on the same team trying to keep patients safe.

Old school nurse here and I've never liked the Pharmacy transition to filling our syringes and sticking a printed out label on it. Im just not that trusting. Give me the vial anyday 😁
