the entire design is wrong , thats an internal SPD short-circuit ,and unfortunately it's not fused as it is supposed to be

Installer didn't tighten the screw properly on the Live wire - Loose wire = high resistance = gets hot like a heater ( P = I²R ) - It heated up slowly for weeks/months - you won't see it trip - Finally the plastic melted, then arc, then FIRE - The breaker didn't trip because it's NOT overcurrent, it's a bad contact
