I just googled why are cognitive tests given and the answer was to monitor mental decline. President Trump seems to think it's to show how smart he is.

President Trump took the Montreal Cognitive Assessment (MoCA), which takes about 10 minutes to complete. The test includes drawing a clock, naming animals, and repeating a short list of words. Trump stated that the test is tough and that getting a 30 out of 30 proves he has "extreme intelligence". Doctors and medical experts state that the MoCA is not a measure of intelligence. The exam is a basic screening tool used to find early signs of memory loss, dementia, or Alzheimer's disease. A score of 26 to 30 is considered normal. Medical professionals say a perfect score shows normal brain function rather than exceptional brilliance.

The Montreal Cognitive Assessment Test is only given to people showing signs of cognitive impairment. Giving it once is to help diagnose. Giving it multiple times means the person’s decline is being monitored.
