Mel works extensively with couples preparing for IVF, navigating unsuccessful cycles and trying to understand what they can optimise before going into their next round. Because IVF is incredibly sophisticated, but ultimately we are still working with the biology of the egg and sperm going into that cycle. For women, Mel may look at factors including nutrient status, metabolic health, inflammation, oxidative stress, thyroid function, reproductive hormones and the broader health environment in which follicles are developing. And one of the things Mel is particularly passionate about is not forgetting the male partner. Sperm contributes 50% of the DNA to an embryo. Standard semen parameters such as concentration, motility and morphology are important, but they don’t always give us the complete picture. Depending on the case, there may be reason to investigate other factors, including oxidative stress and sperm DNA fragmentation. Then we have implantation. Creating an embryo is one hurdle. The next is creating the best possible environment for that embryo to implant and continue developing. This is where Mel’s approach to fertility care is so valuable. She looks at the whole fertility picture, the pathology, previous IVF cycles, both partners, underlying health factors and what may be modifiable before the next cycle.
