Nurse in belgium here. We absolutely do not administer TPN true the port a cath. It ruins the port a cath, its very sticky en can cause blockage. So if you're up to it, get a Picc line in. Please, wil save you a lot of trouble

The hospital will fast track this and you can also see if you have any reactions. TPN is a very high protocol medication so, starting this in the hospital is a great idea.

If you get on home health for TPN you may be able to add your regular fluid infusions to that. Just a thought to ask whoever the ordering doctor is to also send an order for IV bags and if approved they’ll send it all as well as a nurse to your house for accessing your port and stuff when you need. You get to save that energy for regular life instead of clinic visits!

TPN is sticky like others are saying and can cause blockages, I would also suggest a PICC if I was you. Especially if you use the port-a-cath for other things regularly. You don't want to lose that access

Hope this doesn’t come across as rude, but I got a bit confused when he said ‘if it gets to a point where your cancer comes back’. Do you currently have cancer, or are you battling the aftermath of it like the fact you have no stomach and the bowel issues? Thanks.

Just an RN here, but I would absolutely recommend going to the hospital. Based on this conversation, you’ve already had significant changes in one week… please don’t wait for it to get even worse. The hospital can, and should, get the ball rolling on this and hopefully get you some sort of relief. Respectfully, improving your quality of life isn’t worth waiting for. Wishing you the best!

I’m confused by the doc saying you redress a PICC line every day. I’m a home health caregiver and I’ve only seen weekly dressing changes, not daily (I’m not a medical professional, that’s just what I observed)

As a nurse I’m not a specialist in cancer or colostomy but I do know about nutrition and NG and TPN. Considering your blockage issue I don’t see how well NG feed would do rather than TPN. NG feed is mainly calories to upkeep your body whereas TPN would be the full package and I don’t know why they wouldn’t have referred you for this off the bat when your appetite has been so poor and weight is a big issue. I hope you get things sorted soon🤞🏻 ❤️

As someone who deals with bowel obstructions, TPN is a good call. I was on it for about 2 weeks and it helped my gut heal after my surgeries.
