Aortic aneurysm vs. dissection is a crucial distinction here! The AP chest X-ray overestimates mediastinal width, so relying on it for aneurysm/dissection is risky. Given the sudden onset, neck/gum radiation, and hypertension, acute dissection is the top "can't miss" DDx. Proceed straight to CTA! Cheers! ❤️✋️🙂
