Multiple additional code violations here. NEC 406.5 requires the use of 8-32 machine screws.The 2” plate screw most likely destroyed the receptacle 🤦🏻♂️

Electrical Inspector :
nfpa 70 2023 code violation 314.20. requires a box extension if gape is more the a 1/4 of an inch. As a AHJ i would fail and require a box extension do to life safety issue. this creates a fire hazard.
