Grade 1 to Bone on Bone: What Your Knee X-Ray Really Shows Knee arthritis is graded 0 to 4 on the Kellgren-Lawrence scale, a system in use since 1957. Cartilage itself does not show up on an X-ray, so the grade is built from what does: the gap between the femur and tibia, bone spurs, and how the bone underneath responds to load. Grade 1 (doubtful): The joint space looks close to normal, with perhaps a tiny osteophyte (bone spur) starting to form. Grade 2 (mild): Definite bone spurs, with the gap beginning to narrow. This is usually where the non-surgical treatment ladder begins. Grade 3 (moderate): Clear joint space narrowing, subchondral sclerosis (hardened bone that shows up white on the film), and sometimes small cysts, signs the bone is carrying a load it was not designed for. Grade 4 (severe): The joint space is gone. Bone on bone, with large spurs and marked sclerosis. One detail matters more than most people realize: the X-ray should be taken standing. Cartilage only tells the truth under load. A lying-down film lets the bones drift apart and can hide how much wear is really there. And the grade describes the picture, not the patient. Among people whose X-rays show knee arthritis, studies report knee pain in anywhere from 15 to 81 percent. Some grade 4 knees are quiet, and some grade 2 knees hurt every day.
