A new BMJ editorial reviewed what we know about GLP-1 exposure during early pregnancy. The most reassuring finding is that the largest human study to date did not show an increased risk of major congenital malformations after first-trimester exposure. But that's not the end of the story. Researchers still don't have enough long-term data to know whether there are effects that appear later in childhood, which is why current recommendations still advise stopping GLP-1 medications before pregnancy when possible. One point that doesn't get talked about enough: GLP-1s may actually increase fertility in some people by improving insulin resistance, restoring ovulation in PCOS, and, for some medications, temporarily affecting oral contraceptive absorption.
