He wasn’t fantasizing about her. He was probably jarred because she represented the first sense of normalcy that he can remember for who knows how long. Clean clothes, not covered mud, sand, or blood, warm smile, etc. Imagine going through an unimaginable hell and all of a sudden then a pretty girl comes up and give you a cup of water/coffee and says “welcome back”
