Portable CXR can be very misleading for aortic dissection! ⚠️While it may show a widened mediastinum (which AP technique exaggerates anyway), a normal or inconclusive CXR does NOT rule out dissection. High clinical suspicion + severe radiating pain = Immediate CTA is mandatory! (Also noting the PPM/ICD in the left chest). As a case study, this is really great! Congratulations! In a critical real-life situation, I like to stick with my down-to-earth, structured approach—which is what I based my previous comment on! Greetings & Cheers! ❤️✋️🙂
