ME/CFS has long been dismissed as lacking objective biomarkers, partly because standard bloodwork appears normal. New proteomic research reveals an extensive, measurable biological signature. A 2026 study by Hoel et al. in Cell Reports Medicine (University of Bergen) used aptamer-based serum proteomics to measure 7,326 protein targets in 50 ME/CFS patients and 29 healthy controls. They found 845 proteins significantly different after false discovery rate correction. The pattern showed a broad increase in secreted proteins, a decrease in intracellular proteins (notably from skeletal muscle), and immune reprogramming including a reduction in neutrophil-associated proteins, pointing to coordinated immune, vascular, and metabolic dysregulation. This is a discovery study, not a validated diagnostic test. The cohort was relatively small (79 total), and the authors emphasize that the findings need replication in larger, independent groups and caution against over-interpreting single proteins.
