@nourishingapothecary: If you haven’t had a period for months, I don’t want you to just wait and hope it comes back. Amenorrhoea, the absence of a menstrual period, is a symptom. The important question is why has ovulation and menstruation stopped? There are multiple possibilities. You may be experiencing hypothalamic amenorrhoea, where inadequate energy availability, significant exercise, weight loss or physiological stress can suppress signalling from the hypothalamus to the ovaries. PCOS can cause chronic anovulation, meaning ovulation isn’t occurring regularly enough to produce predictable menstrual cycles. Elevated prolactin can interfere with the reproductive hormone axis. Thyroid dysfunction can affect ovulation and cycle regularity. Coming off hormonal contraception can also reveal an underlying cycle problem that was previously masked. And depending on your age and history, we may also need to investigate ovarian function and conditions such as primary ovarian insufficiency. This is why when someone tells me, “I haven’t had a period in a year,” my first thought isn’t simply what can we give you to bring your period back? It’s why did it disappear in the first place? That means looking properly at your health history, menstrual history, medications, nutrition, exercise, stress and relevant pathology rather than treating amenorrhoea as a diagnosis itself. And importantly, going for prolonged periods without menstruating warrants appropriate medical investigation.
