(Inpatient RD here) VERY glad you’re getting admitted as they start TPN. Given how little you’ve been eating + weight loss, definitely concerned for refeeding risk and it’s beet for you to have your electrolytes checked closely and replaced as needed. Make sure to ask the RD that sees you when you’re admitted all the questions you have 🫶🏻

I dose TPNs in the hospital. Usually we keep patients for 4-5 days to slowly citrate up and balance electrolytes. Every hospital is different but it will likely take more than a day or two.

As an oncology registered dietitian with a certified nutrition support specialist (CNSC) certification, it’s interesting that working with a dietitian wasn’t brought up in this conversation! Dietitians provide guidance on improving nutrition and managing side effects of cancer treatment. We also often provide recommendations on refeeding risk and TPN management to help guide these decisions. We are also your nutrition advocate on your medical team! Consider asking your medical team if there is an option to start working with a dietitian on a regular basis. 🫶

It shouldn’t be this hard to get on TPN 😭 it’s shocking how against it all the doctors are. Sure it has risks just like anything else but in your case (& a lot of other peoples case) the benefits far outweigh the risks. Sooo many cancer patients suffer & die from malnutrition rather than their cancer itself bc nutrition is so often put on the back burner. TPN can save your life!!! Xo an oncology dietitian

RD here - I’ve had patients walk into Emergency will plan to start tpn and get turned away. It’s been so hard to find doctors willing to direct admit when it’s really needed…so what a great game plan!! Rooting for you!

Not all doctors like questions. When my daughter was in the hospital with RSV we asked tons of questions and seemingly pissed off some doctors… all but one attending who was so patient and respectful with us. I don’t know how that all would have ended up if it wasn’t for her buffering for us.

Traveling with TPN is also something you will need to coordinate with your home health pharmacy. It has to stay refrigerated and you’ll be setting it up daily. So all things to learn and consider while inpatient before you go out of town. Here if you have any questions with TPN or even SIBO.
