System error clearly. Techs shouldn’t be allowed to override without a pharmacist signing off. That hard stop would’ve been the first notification of an error occurring which would’ve likely prevented this

Why are we blaming “the system” for the error? The tech purposefully bypassed the safety feature that is in place to stop errors. The system didn’t fail, the techs and pharmacists did. This is like me cutting the brake line on my truck. Then getting mad at Ford when it doesn’t stop and I hit a tree

I've been a technician for entirely too long. Barcode scanning and photographs at each step of the compounding process. The pharmacists verifying should be viewing each photograph. This was wholly avoidable.

Hospital administration needs to be held accountable. Theyre always quick to throw any staff under the bus but they are the ones creating poor work conditions. More work, higher acuity, less staff. How many more have to be harmed for something to change.

This is ultimately hospitals using low paid low education level staff to perform a vital function that a pharmacist should be doing. That pharmacy tech may now face legal consequences, do they even have liability coverage for that position? My guess is that the Buccees gas station pays as much as that tech makes and they are responsible for anesthesia meds. Insane.

As a CRNA, I have always drawn up and prepared my own spinal meds. I can’t imagine why pharmacy would be doing that
