But one of the biggest fears I hear from patients is actually: “What if I don’t wake up?” Especially if you’ve never had sedation before, the idea of being “put under” can feel much scarier than the colonoscopy itself. But here’s something many patients don’t realize: sedation for a colonoscopy isn’t one-size-fits-all. There are generally three approaches: No sedation at all. Yes, a colonoscopy can be performed without sedation, and this is more common in some parts of the world. Moderate or “conscious” sedation. Medications such as midazolam and fentanyl may be used to help you relax and stay comfortable without the same depth of sedation. Deeper sedation with propofol. This is commonly used because it works quickly and patients generally wake up quickly once it is stopped. And remember: for a routine colonoscopy, we’re usually talking about a relatively short procedure — often around 15–30 minutes.
