As a Doctor of Physical Therapy working in home health, I’ve had plenty of patients stop me during an exercise and say: “Did you HEAR that?” Click. Clunk. Pop. And suddenly they’re worried something is wrong with their new knee. The reassuring part is that clicking and clunking sensations are pretty common after total knee replacement. Remember, this isn’t your natural knee anymore. You now have prosthetic components designed to move and articulate with one another. Sometimes what you’re feeling or hearing is simply the mechanical interaction of those components during movement. It can even feel like the prosthesis is “hitting” or “knocking” together. Swelling can play a role too. Early after surgery, the knee can be significantly swollen. As that swelling changes and gradually decreases, the way the knee feels during movement can change as well. You may suddenly notice clicking or clunking that you didn’t notice before. At the same time, the muscles surrounding the knee are still recovering. As your strength and control improve, the way the joint moves can continue to change. For some people, the clicking becomes much less noticeable with time. Others continue to occasionally hear or feel their knee even after they’ve fully returned to their normal activities. That doesn’t automatically mean something is wrong. But I also wouldn’t tell someone that EVERY click should simply be ignored. If the clicking is accompanied by new or worsening pain, significant swelling, instability, locking, a sudden change in function, or something simply doesn’t feel right, that’s when I want you communicating with your surgeon or rehabilitation team. Your new knee may never be completely silent. Sometimes hearing or feeling those artificial components is simply part of having an artificial joint.
