Definition هو مجموعة من 3 inflammatory reaction patterns في جلد/أغشية القطط. تتميز بوجود eosinophilic inflammation بدرجات مختلفة. غالبًا مرتبطة بـ hypersensitivity reactions، وأشهر trigger يجب البحث عنه هو flea allergy، ثم food/environmental allergy. ليست مرضًا معديًا، وليست بالضرورة مرضًا له سبب واحد محدد. --- 2. Pathogenesis Allergen / Trigger ↓ Hypersensitivity reaction ↓ Recruitment of eosinophils إلى الجلد/الأغشية ↓ Eosinophil degranulation وإطلاق inflammatory mediators ↓ Tissue inflammation + collagen/tissue damage ↓ 3 reaction patterns: Granuloma / Plaque / Indolent ulcer --- 3. الأنواع الثلاثة ① Eosinophilic Granuloma Firm, raised lesions ممكن تكون linear أو nodular/plaque-like. أشهر الأماكن: Caudal hind limbs,,Chin Lips,,Tongue/oral cavity Paw pads --- ② Eosinophilic Plaque Raised, red, well-demarcated غالبًا moist/erosive. Very pruritic ← أهم كلمة. الأماكن المشهورة: Ventral abdomen،،Inner thighs Inguinal region. --- ③ Eosinophilic / Indolent Ulcer "Rodent ulcer" Ulcerative lesion Well-demarcated. غالبًا على: Upper lip غالبًا مش شديدة الحكة وممكن تكون غير مؤلمة. 4. Diagnosis A. Clinical examination شكل ومكان الـlesion يعطيان suspicion قوي. B. Cytology مهم Impression smear أو FNA. Diff-Quik stain. نتوقع:Numerous eosinophils مع inflammatory cells بدرجات مختلفة. ولو فيه secondary infection ممكن نشوف neutrophils + bacteria. C. البحث عن الـtrigger خصوصًا: Flea examination/control ثم حسب الحالة: Food trial → environmental allergy investigation D. Biopsy مش لازم لكل حالة typical، لكن لو الـlesion atypical أو persistent/refractory نعمل biopsy لتأكيد الـhistopathologic pattern. --- 🔬 شكل الـCytology Eosinophils خلايا كبيرة نسبيًا. Bilobed nucleus واضح. Cytoplasm يحتوي على bright pink/red cytoplasmic granules مع Diff-Quik. ممكن تشوفي eosinophil degranulation والخلفية فيها proteinaceous material/debris. ولو secondary bacterial infection: Eosinophils + neutrophils + cocci ممكن تظهر معًا.
